Get a fit note
If you require an initial or subsequent sick note you will need to complete an e-consult form by following:
Please leave a message giving the dates which need to be covered and ensure that you allow ample time for the GP to call you to discuss your condition if required.
You only need a doctor’s note if you are unable to work and are ill for longer than seven calendar days. Your employer will provide you with an SC2 self-certificate form for shorter periods of illness. If you have been absent from work for less than seven days and your employer still insists on a sick note the doctor may issue a private note for which a fee will be charged.
If it is anticipated that you will need to be absent from work for seven or more days, your GP will issue you with a fit note following an appointment and may request a review appointment for some patients. Subsequent notes can be arranged over the telephone – please leave a message for the GP giving the dates which need to be covered and ensure that you allow ample time for him/her to call you to discuss your condition.
